MINI-GRANT REQUEST

APPLICATION PROCESS

BACKGROUND

Thank you for requesting an application for a funding request from the Lee County Medical Society Alliance Foundation. As a Foundation, we have been raising funds to benefit those organizations that provide services to our community directly related to health education or the treatment, prevention or rehabilitation of disease, illness or injury.

There are two (2) separate and distinct funding requests. You may request an application for one or both, however, a separate and completed application must be submitted for each one by the official deadline. Each funding request has different eligibility requirements. An organization may not receive more than one award of funds for any given year. The funding requests are as follows:

1. Request to be a MAJOR RECIPIENT. (One major recipient is chosen per year and if a major fundraising project is held, will receive 70% of the net proceeds not to exceed fifty thousand dollars ($50,000.00). Deadline is MAY 1 of the year prior to the fundraising event.

2. Request to receive a MINI-GRANT. (If a major fundraising project is held and depending on the funds remaining after the award of proceeds to the MAJOR RECIPIENT, specific grants, not to exceed ten thousand dollars ($10,000.00) per request will be awarded. Deadline is DECEMBER 1 of the year of the fundraising event.

** DECISIONS ON FUNDING REQUESTS WILL BE MADE AT THE SOLE DISCRETION OF THE LCMSAF. THE LCMSAF RESERVES THE RIGHT TO REFRAIN FROM MAKING ANY CONTRIBUTIONS IF A CHARITY PROJECT IS NOT HELD OR SUBSTANTIAL FUNDS ARE NOT RECEIVED.

** THE LCMSAF BOARD OF DIRECTORS RESERVES THE RIGHT TO ELIMINATE A CANDIDATE FROM CONSIDERATION AS A FUNDING RECIPIENT IF THEY BELIEVE THAT SELECTION OF THIS CANDIDATE WOULD BE DETRIMENTAL TO THE INTERESTS OF THE LEE COUNTY MEDICAL SOCIETY ALLIANCE, THE LCMSAF OR THE CHARITY BALL.

** THOUGH NOT DETERMINATIVE, THE LCMSAF WILL CONSIDER THE NUMBER OF TIMES AN ORGANIZATION HAS RECEIVED FUNDS AND THE AMOUNT OF THOSE FUNDS.

 

 

 

 

 

 

 

 

 

 

 

APPLICATION POLICY AND PROCEDURE

1. The official LCMSAF application form must be used. No additional documents are to be submitted. All entries will be judged based on the information submitted in the body of the application.

2. Each question should be answered and the application form must be completed in its entirety. All requested documentation must be submitted with the application. To ensure the fairness of the process, we are unable to call you to request missing information or documents.

3. A completed and separate application must be received for each funding request. As there are totally different procedures and judges for the two categories of funding requests, we are unable to share or transfer applications.

4. Other than the application, no communication or discussion by the organization, it’s members or through alliance members should be initiated regarding the merits of the organization and/or it’s project.

5. Applications that are hand delivered, emailed or faxed will not be accepted.

6. Completed application and attachments should be postmarked by December 1 and sent to:

7. All applications and attachments become the property of the LCMSAF and will not be returned.

8. The LCMSAF relies in good faith on the representations and facts contained in the application. If there is any change in the information contained in the application, the applicant has an affirmative duty to notify the LCMSAF in writing of the specific change(s). If the LCMSAF is notified of possible changes from a source other than the applicant, the LCMSAF will request clarifying information from the organization.

9. Irregardless of how the change in information is discovered, the LCMSAF reserves the right to reevaluate this change in information to determine what effect, if any, these changes will have on the funding request, including but not limited to, no changes in the funding approval, a change in the amount allotted, the withdrawal of funding approval or the request for a return of funds received.
