Financial Information
While Surgical Associates of Southwest Florida does accept Medicare Assignment, we will transmit your charges electronically to Medicare. We will file your secondary insurance after we have received your explanation of benefits. We also file all private insurances.

We do ask that payment be made at the end of your appointment unless other arrangements have been made. If you encounter financial difficulties, we will be happy to set up a payment plan. Surgical fees incurred are the sole responsibility of the patient.

The statement you receive from Surgical Associates of Southwest Florida represents surgical services rendered solely by our office. Other medical professionals who assisted in your care, such as radiologists, pathologists and anesthesiologists, will send you a separate statement for their services.

Due to changing regulations and the many forms to fill out, we know that most insurance matters are confusing. Should you have any questions or problems, please contact our insurance coordinator or financial coordinator.

All Workmen's Compensation patients must have proper authorization from their employer before Surgical Associates of Southwest Florida can provide treatment. This written authorization ensures us that all expenses incurred will be covered.
