After the form has been submitted, please submit $7.00 (US) by check or money order (no cash, please) to the address below. Upon receipt of the money, we will email you a password so that you can directly download the book in rich text format, a format that most word processors can easily handle.

If you prefer, please submit by snail mail a check or money order for $8.50 (the additional $1.50 for shipping and handling) and we will send you the book on a 3 1/2' floppy disk in IBM format. Sorry, we don't have the text in Mac formats ;-(
